The Initial Inquiry
Typically my involvement with a client begins with an inquiry by phone or email. The call may be from any of the following responsible parties: spouse, son or daughter (in or out of state), or an attorney who is legally responsible for an individual. The individual they're calling about is either living alone at home or in a care facility but having problems, has had an emergency and is in the hospital, or, has been or will shortly be released from the hospital back to the home or into an assisted living facility or nursing home.
Responsible parties are relieved to find someone who can quickly respond in an emergency situation, provide answers in a timely manner, oversee care, and monitor needs.
The Response
I will offer to make an on-the-spot visit to define needs, and complete a full assessment. The assessment includes but is not restricted to: current health condition(s), current physicians, current therapies, living conditions, diet and nutritional status, mental health or cognitive impairments, pharmaceuticals (prescribed and over the counter) and compliance, financial preparedness, insurance issues, and legal preparedness.
This information enables me to determine the best options to satisfy identified needs within the framework of an individual's physical, mental, financial and legal abilities. Recommendations to the concerned party will be made.
Services and related fees are discussed with the concerned party, and a contract is faxed or sent by regular mail, signed, and returned to me.
Service providers who may be referred have been assessed and found to meet requirements set forth in my mission statement. That is, "If a service provider isn't good enough for my parents, they're not good enough for yours".
I use and refer to: case tested geriatric physicians, specially certified elder law attorneys, home health providers, assisted living facilities & nursing homes. These service providers call back promptly when I request, and call me to report results of tests, changes in status, or when there is a recommendation.
Continued monitoring and follow up is available and arranged and outlined per contract as desired by the concerned party.
